Being assured that our clients receive the most efficacious care is of the utmost importance to us. To ensure that things run smoothly, we are outlining here the responsibilities of all parties so that everyone is informed as to how the various processes work.
The provider is responsible to do the following:
Verify EAP authorization in the portal before first session. Authorization must be in place in order for payment to happen. If no authorization the client can contact us to get one in place.
You are not allowed to bill the client for EAP sessions.
To request sessions beyond the initial authorization, complete the "Additional Sessions request on the portal" prior to the completion of the last authorized session. Make sure to follow all procedures around this, as shown in this PDF.
All billing to be completed within 90 days of the authorization expiration date through the Provider Portal. You will need to fill in all areas of the session. Keep client informed of progress in treatment.
Contact MINES & Associates prior to making any additional referrals for any other mental health related treatment.
To respond to any emails/calls from MINES clients or staff.
To get a client in for treatment within 5 days of initial contact. If you aren’t able to do this, let the client know and us at MINES so we can put you on hold.

The provider is responsible to do the following:
Verify insurance eligibility of the client prior to initiation of treatment, including exclusions in terms of diagnosis and/or treatment modality.
